For those who are eligible the Centre has government funded programmes of Meals on Wheels, Respite – Centre Based Day Care and In Home, Transport, Social Support, Podiatry, Physiotherapy, Hydrotherapy and Home Maintenance & Minor Modifications Service. These services are explained in more detail further on.
Home and Community Care & Community Care funded services
The following services are available to those who are assessed eligible.
To be eligible to access HACC and Community Care services operated by the Centre, a person must live within the service area of the Centre and
- Be an older, frail person with a moderate, severe or profound functional disability which makes it difficult to perform the task of daily living without help or supervision.
- A younger person with a moderate, severe or profound functional disability which makes it difficult to perform the tasks of daily living without help or supervision.
- A carer of persons specified in the above categories, or
- Would be at risk of premature or inappropriate long term residential care in the absence of basic maintenance & support services.
